Play low from dummy. South will insert C9 as you ruff. Cash HA, draw trumps then start running diamonds. When they don't break, exit with a low heart. South is down to clubs, so dummy's CQ is your 12th trick. Neat. Same on a diamond lead.
Why cash HA early? Get in before South works out to unblock HK. On the actual layout unblocking HK does not help the defence.
